What Is the Difference Between an Ovarian Cyst and Ovarian Cancer?

An ovarian cyst is a structure that develops inside or on the surface of the ovary and is often fluid-filled. It is especially common in women of reproductive age, and many ovarian cysts are benign. Some cysts develop in relation to the ovulation cycle and may disappear on their own over time.

However, not every cyst is evaluated in the same way. The structure of the cyst, the patient’s age, menopausal status, symptoms, ultrasound findings, and blood tests when necessary should be assessed together. This evaluation helps distinguish a benign ovarian cyst from an ovarian mass that requires closer investigation.

Does an Ovarian Cyst Always Mean Cancer?

Most ovarian cysts are not cancer. Simple, thin-walled, completely fluid-filled cysts seen especially in women who menstruate are often considered functional cysts.

Functional cysts may develop during the ovulation cycle. Follicular cysts and corpus luteum cysts are included in this group. These cysts usually do not cause symptoms and often shrink or disappear on their own during follow-up.

On the other hand, some cysts need to be followed more closely depending on their structure, size, the patient’s age, or accompanying symptoms. The important point is not only to say “there is a cyst,” but to correctly evaluate what type of structure the cyst has.

How Can a Benign Cyst Be Distinguished from a Suspicious Mass?

One of the most important methods used in the evaluation of ovarian cysts is ultrasound examination. Ultrasound helps assess the size of the cyst, its wall structure, content, whether it has septations, and whether it contains solid areas.

In general, benign cysts are more likely to be thin-walled, regular in contour, unilocular, and completely fluid-filled. Shrinking or disappearing during short-term follow-up may also support benign features.

In masses that require more careful evaluation, findings such as solid areas, thick septations, irregular structure, rapid growth, bilateral involvement, fluid accumulation in the abdomen, or increased blood flow within the mass may be seen. These findings do not directly establish a diagnosis of cancer; however, they may require more detailed evaluation.

Can Symptoms Help Distinguish the Difference?

Some ovarian cysts cause no symptoms and are detected incidentally during a routine gynecologic examination. Others may cause groin pain, menstrual irregularity, bloating, or a feeling of pressure.

In ovarian cancer, symptoms may often progress more insidiously. Complaints such as abdominal bloating, early satiety, loss of appetite, groin or abdominal pain, frequent urination, and unexplained weight loss should be taken into consideration.

The presence of these symptoms does not necessarily mean cancer. However, gynecologic evaluation should not be delayed when symptoms are newly developed, gradually worsening, or persistent.

Which Symptoms Require Urgent Evaluation?

Ovarian cysts can often be followed. However, in some situations, urgent evaluation may be necessary. If sudden and severe abdominal or groin pain, nausea, vomiting, fever, faintness, or progressively worsening pain occurs, a healthcare institution should be consulted without delay.

This situation may sometimes be related to rupture of the cyst or ovarian torsion, which means that the ovary twists around itself. Although ovarian torsion is rare, it is a condition that requires urgent evaluation.

Does a High CA-125 Level Mean Cancer?

CA-125 is one of the blood tests used in the evaluation of ovarian cancer. However, an elevated CA-125 level alone does not establish a diagnosis of cancer.

This value may also increase in endometriosis, fibroids, infections, menstruation, pregnancy, and some benign conditions. Therefore, the CA-125 result must always be interpreted together with ultrasound findings, the patient’s age, menopausal status, and clinical evaluation.

Especially in the postmenopausal period, the presence of an ovarian mass together with elevated CA-125 requires more careful evaluation. When necessary, HE4, ROMA score, advanced imaging methods, or gynecologic oncology evaluation may be planned.

Evaluation Before and After Menopause Is Different

Ovarian cysts are more common during the reproductive years and are often benign. During this period, different benign cyst types such as functional cysts, endometriomas, or dermoid cysts may be encountered.

In the postmenopausal period, newly detected ovarian cysts should be evaluated more carefully. This is because the possibility of cancer in ovarian masses detected after menopause carries greater importance compared with masses seen during the reproductive years.

For this reason, the same ultrasound finding may be interpreted differently in a young patient and in a postmenopausal patient.

When May Gynecologic Oncology Evaluation Be Necessary?

Not every patient with an ovarian cyst needs to be referred to a gynecologic oncologist. However, if certain findings are present, more detailed evaluation is important.

Gynecologic oncology consultation may be necessary especially in the following situations:

  • Ovarian mass after menopause
  • Suspicious appearance on ultrasound
  • Solid area or irregular structure
  • Fluid accumulation in the abdomen
  • Rapidly growing mass
  • Markedly elevated CA-125 level
  • Family history of ovarian or breast cancer
  • Genetic risks such as BRCA or Lynch syndrome

Are Cysts That Require Surgery Cancer?

The recommendation for surgery for an ovarian cyst does not always mean that cancer is suspected. Surgery may be recommended if the cyst is large, causes pain, does not disappear, carries a risk of torsion, or cannot be clearly evaluated based on its structure.

However, if there is a suspicion of cancer, surgical planning is performed differently. In such a case, the aim is not only to remove the cyst, but also to correctly assess the spread of the disease and plan the necessary surgical approach properly.

What Should Be Considered During Follow-Up of an Ovarian Cyst?

In cysts for which follow-up is recommended, ultrasound control is usually performed at certain intervals. During these controls, whether the cyst has decreased in size, whether its dimensions have changed, and whether any new suspicious findings have developed in its structure are evaluated.

The patient’s symptoms are also important during follow-up. If symptoms such as significant abdominal bloating, increasing pain, early satiety, loss of appetite, unexplained weight loss, or bleeding outside the menstrual period occur, the follow-up plan should be reviewed again.

To understand the difference between an ovarian cyst and ovarian cancer, looking at a single finding is not sufficient. Ultrasound findings, blood tests, the patient’s age, menopausal status, and symptoms should be evaluated together. Not every ovarian cyst is cancer; however, some cysts may require regular follow-up or gynecologic oncology evaluation.

In summary, when a cyst or mass is detected in the ovary, the most appropriate approach is to perform an individualized evaluation and determine a suitable follow-up plan.
